Patient Questionnaire
Please follow these instructions to complete your patient questionnaire:
- Visit http://www.nutritionworkshop.com/support.asp.
- Click the Download the Remote Patient Questionnaire link.
- Install the Patient Questionnaire software (Drug-Nutrient_Patient.msi).
- After installing the software, locate the Drug-Nutrient Patient shortcut
on your desktop. Double-click on the shortcut to launch the software.
Once the Patient Questionnaire has been installed, simply enter basic information
about yourself and your current drugs and supplements into the program. After
completing the process, you then have the option of emailing that information
directly to Meridian, or you can print a report to bring to the office. Alternately,
you can copy the report file to a diskette or CD and bring it with you to the
office.
